review of your current diet compared to your estimated needs
education and counselling on how to change your diet to meet your nutrition needs and reach your best health
written copy of your nutrition plan, including recipes, resources, and specific recommendations discussed

monitor progress towards goals
ongoing education and counselling to reach your nutrition and health goals
written notes post session
may include letters to doctors or for nutrition funding
may include menu planning, recipe, and grocery lists as desired

initial or follow up appointments completed in the home setting are an Excellent option for:
those who prefer face to face appointments
those who have difficulty going out due to mobility, health, mental health, or cognition challenges
families or caregivers wanting to learn as a group
those with family members who live afar wanting to attend virtually in order to best support independent living and aging in place
